Required documents:
- Operating Authority, License, and Permit with MC Number
- Proof of Liability & Cargo Insurance
- $1,000,000.00 Minimum Liability
- $100,000.00 Minimum Cargo
Note: Certificate(s) listing TLC as the certificate holder.
(Contingency Insurance is not acceptable)
- Substitute W-9 form with Federal Identification Number
Please note: The Substitute W-9 form is an internal form used during the initial approval and set up process and can be used in conjunction with your original W-9 form, but the original W-9 can not be used in place of the Substitute W-9.
- Current DOT Safety Rating and date rating was received
- Master Transportation Services Agreement - Signed & Dated. (TLC supplied)
Additional documents are required upon request:
- Hazardous Material Certification. (Optional unless transporting HM - any quantity)
- Current Rate Schedule
- Accessorial charge schedule
- Fuel surcharge tables
